Test Reservation Request Form

Students with a verified disability are expected to meet with instructors at the on-set of the term to make arrangements for appropriate academic accommodations. An instructor can make arrangements for test accommodations at the Testing Accommodation Center (TAC) by filling out the online Test Reservation Request Form.
