Dean's Recommendation Form (PDF)
Dean’s Recommendation forms are sometimes required by professional schools or professional associations, as well as by prospective employers. The forms usually say something like: “To be filled out by college dean or official.”

Petitioning the Academic Standards Board (PDF)
Exceptions to college policies may be granted only upon written petition to the Academic Standards Board.

Students who have been absent from the College for more than two full years (24 months) apply for readmission by submitting this form. International students on temporary visas who have missed a full year must reapply through Undergraduate Admissions

Out-of-residency information: Pre-evaluation
Website for pre-evaluation of credit. In general, academic courses from an accredited institution are transferable. Please note that this form is a pre-evaluation of the course. An official evaluation is done upon receipt of an official transcript. You may consult your advisor regarding distribution or concentration requirements.
