To
register for the 37th Annual Meeting of the Society for Mathematical
Psychology, complete the registration form

The registration fees for the Annual meeting are:

The registration fee includes all conference session, refreshment breaks each
day, the Thursday evening opening reception, the Friday evening banquet, and
box lunch for Friday and Saturday. Guests are welcome to the Thursday evening
reception and may purchase tickets to the Friday evening banquet for $30 each.
To
participate the tutorial workshop and/or the Falmagne Festschrift, please check the appropriate boxes in
the registration form and pay the extra
fees -- $20 for the workshop and $80 ($60 for students) for the Festschrift.
Guests may purchase additional Festschrift banquet ticket for $60 each.

Requests for refunds will be honored if received in writing by June 28, 2004. 50% refunds will be honored for requests made between June 29 and July 28, 2004.
On-site registration is Thursday, July 29 from 8:30am - 11:00am in the East Hall atrium and from 6:00pm - 9:00pm in East Hall. Friday morning registration is from 7:30am - noon in the East Hall atrium.
Individuals with special requirements to accommodate a motor or sensory impairment should indicate their needs in the space provided on the registration form. Information must be received by June 28, 2004.
