The MELAB Official Identification Form is required when you arrive at your test center. Fill out the form by typing the requested information into the required fields. You should be able to type directly into the form on your screen.
Then print the form and mail it or bring it to your MELAB test center. Be sure to attach a recent photograph to the form.
