When a student accepts your offer to work with you, the student will need you to sign their fall contract. Then the student will return their signed contract to the UROP office to get permission to register for Fall semester.
Use this form to add or change project staff members who can sign your UROP students timesheets, abstract, and other UROP documentation. You can fax this form to 734.615.9971, Attn: Front Desk Staff.
Your student should bring you a winter contract / student evaluation. The form will be pre-printed with the student name, mentor name and project title. The purpose of the student evaluation is to promote feedback between the mentor and student.
This Sample Form is for the Fall 2012 Semester only. Students should have their final abstracts typed into this form, have you sign it and answer the questions on the bottom. Then the student needs to return it to the UROP office by December 5th.
